Case Management
At Nurture Nest Play Therapy and Counseling, we understand that accessing necessary services and resources can be challenging for families dealing with mental health or substance use disorders. That's why we offer targeted case management services to help our clients navigate through these difficulties and access the resources they need to improve their daily lives.
Our Targeted Case Managers work closely with families to identify and assess their needs, and help them to access a variety of services and resources. This may include assistance with locating and securing housing, linking to employment opportunities, and referring clients to mental health or substance use services such as individual, group and/or family therapy, AA, NA, parenting classes, and more.
We also help families to navigate social support services such as SSI, Disability, GED classes, College Applications, Food Stamps, and more. Our Case Managers collaborate with our team members, including therapists, community support providers, primary care physicians, psychiatrists, nurse practitioners, social services, the department of juvenile justice, teachers, and anyone else who may be supporting the client.
In addition, our Targeted Case Managers assist with crisis planning for times when life becomes challenging. We help families to identify and link with other support services such as the Michele P. Waiver, SCL Waiver, and more, and we monitor the quality and access to therapeutic and other needed services.
Our Case Managers meet with clients several times monthly to ensure that their needs are being met, resources are being utilized, services are going well, and to continually assess their needs. Medicaid recipients who have a mental health or substance use disorder and need help accessing services and resources for social, medical, educational, vocational, housing, or other needs that impact the mental health or substance use challenges affecting daily living are eligible to have a Targeted Case Manager.

Case managers can do a wide range of tasks to support their clients. Some of the things that case managers can do for their clients include:
Conduct a thorough assessment of their clients' needs and help develop a comprehensive care plan.
Provide education to clients and their families about their condition and available resources.
Connect clients with medical, mental health, and social services that are appropriate to their needs.
Advocate for their clients' rights and help them access available benefits and entitlements.
